Welcome to Cub Scout Pack 989

Cub Scout Pack 989 is chartered by the Mill Creek Elementary Parents for Scouting. The Pack was formed in September, 1989, when Mill Creek Elementary School was first opened and continues to serve boys in grades 1 through 5 who attend the school or live in the general area.

	Youth BSA Application Form Families interested in joining Scouting can download and complete this Youth Application form. Bring the completed form to our next Pack meeting or contact Pack leaders to get started sooner.
	BSA Health and Medical Record Form This electronic health form is for youth and adults. Sections A and B must be filled out for any Scout camp activity. Section C must be completed a) if the adult is over 40 years old, or b) the Scout is camping 72 hours or more.
